Rebecca Dodd (f) Pharmacist

Pharmacists are becoming more involved in the care of patients in General Practice. Having the pharmacy team working in our surgery provides valuable medicines support, a close link with our community pharmacy teams and allows our GPs to focus their skills and time where they are most needed for patients who have more complex needs. Pharmacists are experts in medicines and can talk to you about how to get the best from your medicines

Care Coordinator

A care coordinator will support you to be actively involved in managing your health and to make decisions about your care needs and treatment. They will help to coordinate appointments and ensure any information about your health needs is in a format that you can understand. They will support you to make shared decisions about your health and care needs and support you to communicate these choices to other professionals when needed.

Melody Gibbon (f) Social Prescriber

Social prescribers (also called social prescribing link workers) work together with GP practices to ensure that the wider needs of patients are addressed. They connect patients to groups, activities and services in their communities to meet their social, emotional and practical needs that affect their health and wellbeing. They are the bridge between your GP and all the non-healthcare services available in your community.

Accept referrals for patients over the age of 16yrs with the following conditions:

  • New MSK condition within the last 3 months
  • Acute flare up of a chronic problem (including OA)
  • New soft tissue injuries
  • Possible muscle/ligament/tendon or bone issues
  • Spinal related or spinal pain
